Can the Maryland Medical License fees be charged to an NIH grant if you need to have it as the director of the grant? How about malpractice insurance fees?

Yes, those costs can be charged to an NIH grant, however, they may only be charged to the grant as a direct cost and the cost assigned/charged to individual grants must be based on the manner in which the insurer allocates the risk to the population covered by the insurance.
Said another way, you must determine how much of these costs are directly attributable to the grant in order to charge to that grant.
